Recreation Resource Guide

To participate in any of our programs you must submit our Application Form. It will remain current for 2 years.

After the two years, you only need to update information already on file by filling out the Application Update Form

Please print the application, fill it out completely and send it to our STRIDE office:

After submitting the application, you will be added to our database to receive our mailings. You are responsible for reserving a spot in any of our programs. Follow instructions on our website, or call our office (518) 598-1279.
